This document is an addendum to MKULTRA subprojects 8 and 10, detailing administrative responsibilities between a contractor and sponsor for research conducted from June 9, 1953, to September 15, 1955, at a Boston hospital.
This is one record. The archive holds the rest — ask it anything across the MKUltra files and every answer is cited to the page.

The image is a screenshot of a website or digital document, not a scanned page from a physical declassified CIA document as implied by the surrounding text. It displays a stylized graphic of a vault door on the left side, with a blue glow emanating from it. On the right, white text is overlaid on a black background, with the title "THE BLACK VAULT" in a stylized font at the top. The text explains that the document was obtained from The Black Vault, an online database of declassified government documents, specifically from the MKULTRA/Mind Control Collection. A yellow hyperlink "http://mkultra.theblackvault.com" is visible at the bottom of the text. The image does not contain any photographs of people, locations, equipment, handwritten annotations, signatures, official stamps, forms, diagrams, or tables. There are no visible redactions or visual evidence of experimental procedures.
This image contains a page from what appears to be a ledger or accounting form, likely related to a project named "MKULTRA Subproject 8." The page is dominated by a table with columns for "scription," "Date," "Obligations," "Expenditures," and "Balance." There are handwritten entries in these columns, including dates like "June," "Aug," and "Feb," along with numerical values for obligations, expenditures, and balance. The top right corner has a handwritten annotation "8-1." There are no photographs, stamps, diagrams, or evidence of experimental procedures visible on this page. The content appears to be purely administrative and financial record-keeping.

This page is a black and white scanned document, likely a memorandum. It contains typewritten text including headings like "MEMORANDUM FOR", "SUBJECT", and numbered points detailing an addendum to subprojects. There are several handwritten annotations, notably a date "8-2" at the top right, and possibly initials or symbols within the text. Significant portions of the document are heavily redacted, obscuring names and specific details, particularly in points 2, 3, and 5. A signature is present at the bottom right, belonging to "SIDNEY GOTTLIEB, Chief, TSS/Chemical Division," followed by an "APPROVED" section with more redaction. The overall impression is of an official document from which sensitive information has been intentionally removed.

This document is a declassified CIA record, likely from the MKUltra program, that appears to be an order form or requisition. It features handwritten annotations, including "Prof Ziltea 8-3," and official markings such as "TSS/CD." The page contains several fields with handwritten entries, such as "Room 2409 - Qtrs. Eye," and details about a chemical order, including its name, identification numbers, quantity, and a price. Significant portions of the document are redacted or obscured with black ink, particularly in the "Order From" section and along the margins, suggesting sensitive information has been removed. A printed statement at the bottom indicates the chemical is not for drug purposes, followed by lines of blacked-out text.

The document appears to be a form, likely a receipt or certificate, with official typed text and handwritten annotations. Two certificates are present, both dated March 2, 1954. Each certificate states that the undersigned has received a cashier's check, with the first check for $26,333.33 and the second for $526.67. Both certificates have large, dark redactions obscuring significant portions of the page, likely covering names or other identifying information. Handwritten annotations include "8-4" at the top right and the initials "M.D." below each redacted section, as well as circled letters "B" and "C" in the right margin. There are also a few circular stamps scattered throughout the document.
This page is a memorandum receipt form documenting the acknowledgment of receipt of funds. It includes handwritten check numbers and amounts, along with typed dates. There are handwritten notations, including what appears to be a check number and signature. The form also shows multiple handwritten dates, including "FEB 18 1954" and "18 Feb 54". Redacted content is visible on the form, obscuring a signature. The top right corner has a handwritten notation "8-5".

The document is a declassified memorandum dated February 5, 1954, accompanied by handwritten annotations and the presence of redactions. The top right corner contains the handwritten annotation "8-6." Several sections of the document are obscured by black redactions, particularly in the middle and lower portions, obscuring details about payments, recipients, and potentially sensitive information. The document includes official stamps indicating "RECEIVED" and dates like "FEB 1 S 1954" and "FEB 18 1954," suggesting it has gone through an internal process. The overall visual presentation is that of a typical administrative memo with interspersed handwritten markings and redactions.
A detailed proposal is attached. The principal investigator will serve as a grant for the estimated for this service. Thus the total charges for this project will not exceed $40,290.00 for a period ending September 1965. (Director of the hospital) are cleared through TOP and are aware of the true purpose of the project.

CONTINUATION REQUEST FOR PROJECT III S.N. ACID DIETHYLAMIDE TARTRATE. Includes Study of Antagonists and Related Drugs such as L.S.D. Lysergic Acid Diethylamide, etc. The project for next year is in general a continuation of the L.S.D. project along similar lines to that of this year. Each aspect of the project (biochemical, neurophysiological, sociological, clinical psychiatric) is to be continued. The principal addition to the broad scope of study is that of a Psychological section which consists of work not outlined in last year's protocol but which nevertheless has been going on during the year and has brought forth sufficient findings to warrant its continuation and expansion.
Each scientific aspect of the project has led to the development of procedures and methods which are now of usefulness in the study of lysergic and other similar drug reactions. The further application of the methods which have been developed during the past year of experimentation will be one of the principal focuses of next year's work, such as L.S.D. and any other drugs which make their appearance, and to the study of other drugs in conjunction with L.S.D. with the goal of ascertaining antagonistic effects. We would also like to expand the base of the experiment by applying these methods to the study of patients with acute psychoses not to only test applicability but also
This page is a typed document with handwritten annotations and a few redacted sections. It appears to be a certification form, with three numbered sections each starting with "It is hereby certified that...". There are lines for dates and signatures in each section, with some handwritten entries present, including dates and the letters "A" and "G" written in the margins. A significant portion of the page has blacked-out text, most notably in the first certification section and at the bottom of the page covering a signature line. The document is dated externally at the top right with "8-6". There is also a stamp that appears to say "CERTIFICATIONS".

This declassified CIA document appears to be a financial record from August 31, 1953. It contains handwritten annotations and a numerical identifier "8-7". The main body of the text is a certification of receipt for a cashier's check in the amount of $13,166.67. There are two significant redactions (blacked-out areas) on the page, one obscuring information related to the source of the check and another obscuring further details. Additionally, there are marginalia marks labeled "-B" and "-C" next to text, suggesting some form of internal review or categorization.
The document is a scanned page with a handwritten date "8-8" in the upper right corner and black ink scribbles or smudges present in several areas. A sentence of text states, "This is to certify that today I have received Cashier's check No. M133785 of the", followed by more scribbled-out text and the amount "$263.33". There are also handwritten letters "B" and a circled "C" appearing to the right of the text, and the initialism "M.D." is visible below the scribbled-out content. The bottom portion of the page also contains a large, dark, scribbled-out area.

This document contains two copies of a "Memorandum Receipt." Both receipts are dated August 28, 1953, and are from the "Budget Office, TSS" concerning "Two checks." Both acknowledge receipt of checks totaling $13,430, specifically mentioning check # M133786 for $263.33 and check # M133785 for $13,166.67, as payment for invoice #1, Subproject 8, Project MKULTRA. Both forms include lines for a signature of the recipient and date received, with the lower copy displaying a handwritten signature and date ("August 28, 1953"). There are also black redactions obscuring specific names or departments in the "TO:" field of both receipts. The overall document appears to be a standard administrative form used for tracking financial transactions within the organization.
