The document is a typed memorandum on plain white paper, with a header indicating "Office Memorandum" and "UNITED STATES GOVERNMENT." The top right corner contains handwritten numbers "74" and "5". Scattered throughout the document are numerous black ink smudges and obscured content, indicated by black rectangular redactions, primarily over names and specific details. These redactions obscure significant portions of the text, particularly where names of people or specific locations would be. There are also faint, faded stamps and markings towards the bottom of the page, including what appears to be a circular stamp with some design. No photographs, diagrams, tables, or explicit visual evidence of experimental procedures are present.
