This memorandum from 1975, referencing a 1963 Inspector General's report, defines MKULTRA as the research and development phase for materials used in clandestine operations to control human behavior, and MKDELTA as the system for controlling their operational employment. It notes that covert testing on unwitting U.S. citizens began in 1953, and as of 1960, no effective knockout pill, truth serum, aphrodisiac, or recruitment pill was known to exist.

Below this title, in smaller white text, is a description of the document's origin, stating it was obtained from "The Black Vault, an online database of declassified government documents," and specifically housed in their "MKULTRA/Mind Control Collection." The text further explains that this collection contains over 20,000 declassified pages from the CIA and provides a URL for online access. The bottom line of text, in a yellow font, is the website address: "http://mkultra.theblackvault.com". At the top right, a stamp indicates "15 SEP 1975." There are also handwritten notations in what appears to be blue ink, including the number "413" and some scribbled text. The document is a typed memo with the subject "MKULTRA/MKDELTA," detailing information from an IG Report dated July 26, 1963. No photographs, diagrams, forms, or visual evidence of experimental procedures are present. The text content describes the nature and control systems of MKULTRA and MKDELTA programs. The page contains typed text, with numbered points 7 and 8. There are handwritten annotations at the top right, including the number "413". The text of point 7 discusses a 1960 Gottlieb report regarding Clandestine Services, TSD, and operational capabilities in special drugs and chemicals. Point 8 mentions scarce records and summaries of actual uses. There are no photographs, forms, diagrams, or structured data. No redactions or visual evidence of experimental procedures are visible. The page contains a handwritten signature that appears to read "Donald F. Chamberlain Inspector General," along with an illegible handwritten notation in the upper right corner. There are also several instances of scattered ink marks and smudges across the document, along with some faint printed text at the bottom left that appears to be a page number. A faint vertical line, possibly a ruler mark or fold, runs down the right side of the page, and a horizontal line is visible in the upper right. The document appears to be a scanned page with some artifacts of the scanning process.
