The image displays a black background with a stylized title "THE BLACK VAULT" in a white, graffiti-like font. To the left, there is a detailed, semi-realistic illustration of a vault door, complete with a large dial, rotating mechanisms, and a circular locking wheel. The rest of the page is filled with white text that details the origin of the document: it's from The Black Vault, an online database of declassified government documents, specifically from the MKULTRA/Mind Control Collection. The text also provides a URL for the online collection. There are no photographs of people, handwritten annotations, signatures, official stamps, forms, diagrams, tables, redactions, or visual evidence of experimental procedures.
This document is a typed memorandum. There are several prominent black redactions obscuring text in the subject line, body of the memorandum, and at the bottom of the page. Handwritten marginalia includes a capital "A" near the right side of the page and another capital "A" lower down, also to the right. At the top right, there is handwritten text that appears to be a code "A/B, 3, 5/10". Two "C" shapes are drawn in the margin on the left side of the page. The document has the appearance of an official internal memo due to its formatting.
